We offer online billing through the Employer Connection or deliver by another method. The banking and billing information form is for groups to complete to indicate payment method.

  Download the billing and banking information form here. View the ACH Fully Insured Billing Form, ACH Self-Funded Billing Form, and more on our group forms page.

• Billing on Employer Connection

  You can access bills through our Employer Connection, follow these steps to download your group's statements.

  1. To retrieve a billing invoice, click “Billing Invoices” on the left or “Billing” at the top of the “Employer Connection” screen.
  2. Select the billing invoice, you would like to review.
  3. On the next screen, you can view a summary of your invoice and download a PDF or Excel file. You can also view subscriber adjustments and view active subscribers.
