Introducing Strong Guard, Strong Teeth, a program designed to safeguard student-athletes healthy smiles. Delta Dental of Illinois is giving away free, high-quality mouthguards to middle and high school athletic programs throughout Illinois - completely free of charge. 

How It Works

Athletic coaches, directors, and administrators can request free mouthguards using the form below. No cost or obligation – delivery is direct to your school or facility, while supplies last. 

Mouth Guard Program Rules

• Eligibility
  • This program is open to middle and high school sports teams in Illinois.
  • Only official representatives (coaches, athletic directors and administrators) of the school can enter.
  • Each school can enter once per sport. (For example, “football” is a single sport and can include freshman, JV and varsity teams; “volleyball” is a single sport and can include freshman, JV and varsity teams, etc.)
• Entry Method

  If a school has multiple sports, each sport must submit a separate order form. Schools must complete an online entry form providing the necessary information:

  • School name
  • School address
  • Sport requested
  • Representative’s name and position
  • Contact email and phone number
  • Shipping address (may be different from school address)
• Verification

  Each request will be reviewed for completeness and eligibility. Duplicate entries from the same team will be disqualified.
