How to Find a DeltaCare Dentist:
- If you have a DeltaCare IL plan, you can find a DeltaCare dentist using our online PDF directory. (Please be sure to note if the office is closed to new patients.)
- If you have a DeltaCare USA plan, you can find a DeltaCare dentist by using the Provider Search tool. Choose DeltaCare as the network option to view providers in your area (Please be sure to note if the office is closed to new patients).
- Once you find a DeltaCare dentist, you must let Delta Dental of Illinois Customer Service know your selection so it can be added to the monthly patient roster. DeltaCare dental offices receive a monthly roster of DeltaCare patients from Delta Dental of Illinois based on members’ primary dentist selections.

DeltaCare Forms:
DeltaCare specialty claim form: This claim form is only for DeltaCare specialty claims. DeltaCare specialists submit claim forms for DeltaCare patients. You must have a referral from your primary DeltaCare dentist to receive care from a DeltaCare specialist.

- Can I change my DeltaCare dentist?
Yes. You can change your assigned DeltaCare dentist at any time by calling Delta Dental of Illinois Customer Service at 800-942-3772. However, all changes must be received prior to the 20th of the month in order for your change to be effective for the upcoming month. If you make a change after the 20th of the month, your change will take effect in two months.
- What if I need to see a specialist?
If you need to see a specialist, your assigned DeltaCare dentist must provide a written referral to a DeltaCare network specialist per program guidelines (program guidelines indicate the procedures that can be referred and the protocols associated with such referrals by procedure code).
There is an authorization process for both non-emergency and emergency referrals. Please contact Delta Dental of Illinois Customer Service at 800-942-3772 to confirm the specialist’s eligibility on any referrals provided.
- What happens if I need emergency care and can’t see my DeltaCare dentist?
If you are unable to see your DeltaCare Network dentist within 24 hours, you may visit any licensed dentist for emergency relief of pain only (palliative). Follow-up care must be provided by your assigned DeltaCare dental office.
- When can I schedule my first appointment with my DeltaCare dentist?
Once you find a DeltaCare dentist, you must let Delta Dental of Illinois Customer Service know your selection so it can be added to the monthly patient roster.
Your assigned DeltaCare dental office will schedule appointments once your name is on their monthly roster of patients, which is updated the 1st of each month.
