▼ Enrollment Meetings Material Samples
Delta Dental of Illinois provides welcome kits for your members directly to you either by email or mail. These include an overview of the plan highlight sheet, how to guides for finding a dentist and accessing benefits and claims online and other information on using their dental plan.
ID cards are mailed to members' homes.
Open enrollment offers your group the opportunity to educate and remind your employees/members about their benefits. We offer enrollment meeting materials that can be customized for your group. Here are examples of what we can offer your group for a successful meeting.
We have a host of brochures and flyers that you can use at benefit fairs, enrollment meetings and health fairs. We can also create custom versions of these flyers for your group.
Groups can place check stuffers in employees'/members' paycheck envelopes to remind them to take care of their oral health and visit the dentist for regular check-ups.
We offer a variety of electronic or printed postcard reminders for groups to choose from. We can even provide customized versions.
Groups can feature our oral health focused banners on their Intranet, on employee/member message boards, or anywhere else you want to show the benefits of oral health wellness in the work place. We can even provide customized versions for your group.
▼ ID Cards
Here's an example of a group member dental ID card. This card is for the use of the primary member and his or her covered dependents. Dentists will typically need the following information to confirm a member's eligibility:
Members can print a copy of their ID card through Delta Dental of Illinois' Member Connection. Members can also download our Delta Dental mobile app to access an electronic version of their ID card. The app includes the ability to view dental coverage and claims information, obtain a LifeSmile oral health score, find a dentist and access a cost estimator tool to gauge the cost of common dental treatments in their areas. The Delta Dental mobile app is optimized for iOS and Android devices (smart phones and tablets) and is available for free to download in the Apple App Store or Google Play Store. Simply search for "Delta Dental."
DeltaVision® ID cards are exclusively available in an electronic version. They are not personalized and all that the vision care provider will need to verify coverage is the primary member's name and date of birth. Here's an example of a group member vision ID card. The DeltaVision program uses the EyeMed Vision Care networks. Members and their covered dependents will receive network benefits when they visit a vision care provider in the EyeMed network. Members can access their vision benefit information anytime, anywhere on EyeMed's mobile-friendly member website at eyemedvisioncare.com/member or with EyeMed's free mobile app, which is available to all DeltaVision members. The mobile website and mobile app allow members to access their ID card, view benefit information and find an in-network vision provider, all while on-the-go. To download the EyeMed mobile app, visit the Apple App Store or Google Play Store and search for "EyeMed Members."
Please note: DeltaVision is provided by ProTec Insurance Company, a whollyowned subsidiary of Delta Dental of Illinois, in association with EyeMed Vision Care networks.
